TPP announced two wins for a CCG collaborative working across London.
The CWHHE Collaborative is the working partnership between Central London, West London, Hammersmith and Fulham, Hounslow and Ealing Clinical Commissioning Groups. The collaborative is supported by TPP’s SystmOne and was nominated for the awards alongside the GPs from each CCG, in addition to the Public Health teams from the five boroughs, Diabetes user groups, Diabetes UK, and London Diabetes Strategic Clinical Network.
The Healthcare Transformation Awards held as part of The Health + Care Commissioning Show, celebrate the hard work and success across local healthcare systems. The 2017 event was supported by NHS England, NHS Clinical Commissioners and the National Association of Primary Care.
CWHHE CCG Collaboration won two accolades – Innovation in Diabetes Care and Utilising Digital Transformation to Manage Population Health.
CWHHE’s Clinical Director for Diabetes, Dr Tony Willis, commented that most of the work to transform diabetes care across North West London has been driven by the use of TPP’s SystmOne. It delivers care across 235 practices to a combined population of 1.4 million patients. The shared Electronic Health Record (EHR) across their services has been the biggest influence in enabling the collaborative to deliver coordinated and informed diabetes care to the collaborative’s population.
